Abstract: 

Multiple sclerosis is a debilitating disease of the central nervous system. It affects people of all ages but is more prevalent among 20-40 year olds. Patients with MS can be presented with potentially any neurological symptom depending on the location of the lesion. A quarter of patients with MS suffer from bilateral lower limb spasticity among other symptoms. These devastating effects can be detrimental to the patient's quality of life. Hematopoietic stem cells (HSCs) have been used as a treatment for MS over the past 2 decades but their safety and efficacy has are undetermined. The objective of this study is to evaluate the feasibility and toxicity of autologous HSCs transplantation in MS. A literature search was done from 1997 to 2016 using different keywords. A total of 9 articles, which met the inclusion and exclusion criteria, were included in this review. The type of conditioning regimen and technique of stem cell mobilization are summarized and compared in this study. All studies reported high-dose immunosuppressive therapy with autologous HSCs transplantation being an effective treatment option for severe cases of multiple sclerosis. Fever, sepsis, and immunosuppression side effects were the most observed adverse effects that were reported in the selected studies. HSCs is a feasible treatment for patients with MS; nevertheless the safety is still a concern due to chemo toxicity.

Abstract: 

Hematopoietic stem cell transplantation (HSCT) severely undermines the recipients' immune status and makes them prone to complications following viral infection. Here, we report a 3-year-old boy with mucopolysaccharidosis type VI who acquired SARS-CoV-2 infection after HSCT. The boy was diagnosed with SARS-CoV-2 during the post-transplant period (19 days after HSCT) when dealing with acute graft versus host disease (GVHD). He was successfully treated with remdesivir and tocilizumab and recovered. Well-timed treatment with tocilizumab might reduce the risk of invasive mechanical ventilation and death in patients with severe COVID-19 pneumonia in the early post HSCT period.

Abstract: 

Background and Objectives: Graft-versus-host disease (GvHD) frequently complicates Hematopoietic stem cell transplantation (HSCT). Emerging evidence suggests a correlation between gut microbiota and GvHD risk. This study aims to elucidate the microbiota profiles in HSCT patients before and after transplantation and their association with GvHD. Materials and Methods: This study, conducted from December 2022 to December 2023, involved the collection of 15 stool samples from HSCT patients. Bacterial content was quantified using real-time PCR, while interleukin-6 levels were assessed via ELISA. Results: Among the 15 participants (8 male, 7 female), 9 underwent allogeneic HSCT (allo-HSCT) and 6 received autologous HSCT. In the aGvHD group, there was a significant reduction in the abundance of Bacteroides and Bifidobacterium compared to those without aGvHD. Additionally, declines were observed in Clostridium and Firmicutes populations. The genus Blautia also showed reduced prevalence in the aGvHD group, whereas no significant differences were noted in the uncomplicated group. ELISA analysis revealed that interleukin-6 levels remained within the normal range (30-960 pg/ml) with no significant elevation in the aGvHD group. Conclusion: The study highlights a notable association between alterations in gut microbiota, specifically reductions in certain bacterial populations and the development of aGvHD following allo-HSCT.

Abstract: 

Organ failure, including,liver toxicity, renal failure, and neurotoxicity, are frequent complications following HSCT which can affect the transplant outcome, morbidity, and mortality of allo-HSCT recipients: A retrospective study of 206 allo-HSCT patients was conducted to determine the frequency of organ failure and overall survival in patients receiving allo-HSCT. Liver toxicity, renal failure, and neurotoxicity were diagnosed according to clinical and laboratory records pre and post-allo-HSCT. A total of 33 patients (16%) developed organ failure within 200 days after allo-HSCT. Liver toxicity was diagnosed in 12% of patients, and the median time of its occurrence was 22 days (range: 0-207 days) post-allo-HSCT. Two percent (6 of 206) of allo-HSCT recipients presented renal failure. Renal failure was developed within the median time of 33 days (range: 5-88 days). Neurological involvement occurred in 0. 9% of patients. Among 206 patients, the frequency of complications such as veno-occlusive disease (VOD) and graft-versus-host disease (GVHD) was 1. 4% and 16. 01%, respectively. One-year overall survival of patients who had organ failure was 24%, and the mean survival determined 329±, 99. 58 days. Three-month overall survival of patients who developed liver injury and renal failure were 78% and 33%, respectively. Organ failure remains a common complication in patients who received allo-HSCT. Patients with GVHD and two or multi-organ involvement seem to have lower overall survival.
